Isolated limb perfusion (ILP) with tumour necrosis factor-alpha (TNF-alpha) has been used in the treatment of locally advanced melanoma, but the benefit of this treatment is unclear. We studied the effects of ILP with TNF-alpha in the treatment of pre-established metastatic melanoma. C57BL/6 mice with footpad tumours were randomly assigned to 4 treatment groups: (1) ILP with TNF-alpha (1 and 10 microg) with or without additional systemic administration of doxorubicin (1 mg per kg) and (2) intraperitoneal (i.p.) injection of doxorubicin (1 mg per kg) with or without additional systemic administration of TNF-alpha (1 microg per kg). The footpad tumours were measured every second day and the mice were killed 3 weeks after randomisation. Tumour volume and number of tumours were significantly lower in the mice treated with ILP with TNF-alpha than in the mice treated with ILP alone. ILP with TNF-alpha combined with systemic administration of doxorubicin was more effective in reducing footpad tumour volume and number of tumours than the doxorubicin treatment alone. The amount of necrosis in the tumour was significantly higher in the mice treated with ILP with TNF-alpha than in the mice treated with ILP alone. Nerve damage was observed in the footpad of 8 out of 12 mice treated with ILP with TNF-alpha, but in none of the mice treated with ILP alone. The other treatment groups did not display evidence of nerve damage. ILP with TNF-alpha induced several changes in the skeletal muscle and adipose tissue.
